An ex-real estate agent said ‘hi Helen, I’m really sorry’ before allegedly plunging a knife into the chest of his best mate’s wife at the door of her home in Sydney’s eastern suburbs, a court has heard.

Matthew Brian Ramsay, 46, appeared with a shaven head via audiovisual link in Waverley Local Court on Friday, charged with causing a wound or grievous bodily harm with intent to murder.

Ramsay is accused of attending Helen Coulston’s $4.5million Dover Heights house while she was home alone on August 8.

He is then accused of plunging a 20cm knife into the mother-of-two’s chest, and then swinging at her a second time, but missing, the court heard.

That sparked a fierce struggle between the pair that was interrupted by a tradie who was working on a building site across the road. Ramsay then allegedly fled.

During a bail application, the court was told Ramsay was captured on bodyworn after his arrest saying: ‘I didn’t mean to hurt her, I just meant to scare her’.

The 46-year-old had ingested some 120mg of Valium prior to the alleged incident, the court was told. The therapeutic dose is 40mg.

Ms Coulston was allegedly covered in blood and had her arm in a sling when she was wheeled out in an ambulance.

Mr Ramsay was arrested a short time later at the traffic lights on Campbell Parade in Bondi Beach.

Footage from the arrest appears to show an officer pointing a gun at the 46-year-old while yelling at him to ‘get down on the ground’.

Mr Ramsay told police he hadn’t intended to wound Ms Coulston.

The court heard the father-of-one has been struggling with ‘significant mental health issues’ for years, with numerous past stays in rehabilitation.

Magistrate Ross Hudson determined to release the former real estate agent into the care of a strict residential rehabilitation facility in Port Stephens where he would ‘effectively be under house arrest’.

Mr Ramsay will remain at the facility for at least six months.

He is likely to interact with another well-known inhabitant, former TV personality Andrew O’Keefe, who has resided at the centre for months.
