The 34-year-old has been sidelined since suffering a cruciate ligament injury in training just six games into the current season.
Perisic has remained optimistic of returning to action in the spring and being fit for the Croatian European Championships in Germany this summer.
The move ends his time in the Premier League after joining the north London club on a free transfer from Inter Milan in 2022.
The midfielder-turned-defender has scored once and assisted 14 times in 50 appearances for Spurs.

He now returns to the club where he started his football career, although he never made a senior appearance before leaving for Sochaux at the age of 17.
According to reportsPerisic will receive a monthly salary of just €1 for the rest of this season with the Croatian team, as well as in the next campaign.
He will be able to increase his income through various bonuses beyond his goodwill gesture.
Earlier this month, Ange Postecoglou admitted Perisic had likely played his last game for Spurs and shared his disappointment at not being able to be part of their impressive campaign.
“He has been a significant loss to us.” said the Australian. “I thought he would make a very important contribution this year because of his experience and we saw that in the first few games because of the way he influenced games.
‘But he won’t do that. It’s unlikely he’ll play for us again.”
Perisic had started the season well after returning to a forward position following the arrivals of both Postecoglou and left-back Destiny Udogie.
He assisted Richarlison’s 98th-minute equalizer against Sheffield United in September in his last Premier League appearance, helping Spurs complete the last comeback in the competition’s history.
