Since the crash on May 12 last year, the victim continues to have life-changing physical and mental injuries.
The victim allowed the footage to be released to warn others about the impact of drunk driving.
At Guildford Crown Court, Selwood, who lives in Haslemere, was sentenced on April 28 to two years and three months in prison.
He was also disqualified from driving for three years after being found guilty of causing serious injury by dangerous driving and exceeding the alcohol limit.
The woman described how she can no longer pursue many of her hobbies and is now afraid to go near traffic.
In a statement read in court, she said: “My sense of loss is deep, my faith in my world has evaporated, my sense of hope is gone.
Not only did Selwood knock her off her bike, he continued to ride forward, crushing her under the wheels as she lay on the ground.
Not a night goes by without nightmares. My life has changed significantly since falling off my bike.
‘I used to be outgoing, talk to others a lot, go to church, backpack. I did my own DIY and gardening.
“I was never afraid of traffic as I am now, absolutely terrified with a strong sense that it is going to happen again.
“My biggest fear now is that my left leg won’t hold me up due to weakness, which is more common these days, and my mental state isn’t very good.
‘I’m not smiling anymore. I get overwhelmed by everyday events, unable to deal with multiple issues.’
Sergeant Joe McGregor, Surrey Police Investigating Officer, warned of the dangers of drunk driving.
He said: ‘These images are shocking to watch, but it can serve as a stark reminder of the dangers of those who choose to drive under the influence of alcohol.
“The victim would like to express her gratitude to members of the public who stopped Selwood’s vehicle.
“The victim in this case has lifelong physical and mental scars and has bravely said she wants her case to be a warning to anyone who thinks it is acceptable to drink and drive. That’s not it. You are endangering your life and the lives of others.
“Alcohol limits are there for a reason and if you know someone is driving over the limit or after using drugs, call 999.”
