Colorado Avalanche hockey star Sam Girard has left the team and will undergo rehab for alcohol abuse, anxiety and depression.
In a statement released Friday evening, the 25-year-old announced his decision, four days after leaving the team for then-undisclosed personal reasons.
He said through his agency CAA Hockey: ‘I have made a proactive decision to look after my mental health and am seeking treatment for severe anxiety and depression that has gone untreated for too long and has led to alcohol abuse.
‘Taking care of your mental health is of paramount importance, and I encourage everyone to speak out and seek help if you feel you need it.
“I would like to thank my wife, family, friends, the club, my teammates and the fans for their patience, understanding and continued support.”

Girard’s decision received strong support from his fellow professionals and fans, and he now receives support from the NHL/NHLPA Player Assistance Program.
Earlier on Friday, the defender’s father posted publicly online about his son’s condition, saying that “Sam is in good hands.”
He added: “Our man is a fighter, a great fighter and will always honor our region and all of Quebec. Hockey is still his great passion.’

Girard was drafted 47th overall in the 2016 NHL Draft by the Nashville Predators and subsequently dealt to the Avalanche on November 5, 2017.
He made 76 appearances last season — the second-most of his seven-year NHL career — and had already made 15 appearances this season before leaving the team.
The Canadian last played against the Dallas Stars on November 18 before leaving the team.
